Current 6th grade students and parents - in order to participate in athletics next year, there are some steps you need to follow.

1. Complete a pre-screening medical form - you may get this form from the Middle School Nurse - 607-655-8244.

2. Get a sports physical from the school physician - appointments are made through the Middle School Nurse -
607-655-8244. While a routine grade level physicals may be done by your child's private physician, sports clearance must be performed by the School Physician per state regulations.

3. Complete a Health History Form - No sooner than 30 days before the season begins. You must complete a form for each season. you will get this form from the coach during the sign up meetings or through the summer mail for the fall season. You do not need to get this form from the nurse now.
